Below are two items you will need to print out to get your handpiece to us. You may click on the form image to open the form and type in the information in the blanks, and then print it, or you can “Right Click” and “Save As” a blank form to your computer for printing later.
1.) Trackable, Pre-Paid USPS Mailing Label
You will be directed to the USPS website. Please complete the “Enter Your Address Information” section and follow instructions. Shrink to about 70% before printing. PLEASE RECORD TRACKING NUMBER BEFORE MAILING. (Our office will not have access to the tracking number.)
2.) Repair Form
Please complete and print this form to give us an idea of what is wrong; what you would like done; and if you prefer a free estimate first. Remember, the PDF is fillable.
